In completing this third form, a person’s Username is just your unique name you use to log into any website. Many people, but not all, use their informal first and last name and can include or exclude spaces or capitalization.

If you are a couple, first one of you please complete these three forms as an individual and then have your spouse/partner complete a set of forms for themselves. Because we are an age restricted club, your birth date is required but is kept private from the membership.
